2.Certificate copies of identical kind products and non-identical kind products which certificates are awarded already.
3.Factory registered license(when it is different from last application).
4.Factory inspection/investigation form(while different from last application).
5.Authorized trust deed of agent(if available).
6.Trademark registered license(when it is different from last application).
7.Consistency statement.
8.Product electric scheme.
9.Product assembly drawing.
10.Safety components list(Should indicate certificate number if components passed certification).
11.Main raw material list.
12.Differentia description of different type of product in an identical unit.
13.CB test certificate、CB test report(If applicant has).
