1.How to fill in Application Form for Printing and Impressing CCC Mark(Form I):
1) Applicant:fill in the full name of the applicant company not the person,and the Trust Deed: http://china-ccc-certification.com/365.html authorized by applicant should be submitted if the applicant is not the owner of CCC certificate.
2) Mark Process and position: refer to the real conditions of the applicant.
3) Factory:refer to the factory in the CCC certificate.
4) Factory Code:refer to the factory code of the factory in the CCC certificate,which is issued uniformly by the certification body.Applicant may consult to the certification body if he is not certain about that.
5) Certificate No.,Name of product and model:fill in according to the CCC certificate. Applicant may fill in one Application Form for Printing and Impressing CCC Mark if digits 7 th to 10th of CCC certificate are same.
6) Amount of the Used Mark and Management Code:may/maynot fill in refer to real conditions.
7) Post Address,Postcode,contact person,Tel,Fax and E-mail:fill in the address,postcode,name,Tel,Fax and E-mail of the applicant,so that Permission of Printing/Impressing China Compulsory Certification Mark and append page of Permission of Printing/Impressing China Compulsory Certification Mark will be arrived in successfully.
8) Date of Application,Seal of Company/Signature of Authorized Person:fill in the date of filling in Application Form for Printing and Impressing CCC Mark,with clear seal of applicant and signature of authorized person.

2.How to fill in Application Form for Printing and Impressing CCC Mark(Form II):
1) Drawing for CCC Mark:printing or sticking CCC mark with factory code and marked with dimension scale and color of CCC mark.Note:dimension of CCC mark should be in linear proportion to that of standard CCC mark,no deformation (apart from regulated by GB,eg.wire and cable) and marked with mark type:S,EMC,S&E,F,I and no type.
2) Whole Drawing:sticking whole drawing with CCC mark (nameplate,body or packing drawing).Note: no deformation of CCC mark,clear in mark type.Packing drawing may be attached if it cannot be put in.Submit each design proposal of CCC mark use if more mark process and position in the same product.
3) Certificate No.:fill in corresponding certificate No.,applicant may attach pages if he has many certificates to apply for Printing and Impressing CCC Mark.
