Submitting following documents for CCC Mark Annual Verification:
1. CCC Mark Annual Verification Application Form:http://china-ccc-certification.com/169.html

2. Copy of Permission of Printing/Impressing China Compulsory Certification Mark and append page of Permission of Printing/Impressing China Compulsory Certification Mark.
3. Copy of CCC certificate.
Notes:
1. 4 copies of above documents should be sent to us by post, details see our fax. CCC Mark Annual Verification Application Form should be signed by the applicant, other documents not.
2. Permission of Printing/Impressing China Compulsory Certification Mark consists of 2 pages. Page 1 including information of applicant, manufacturer, factory and certificate No.(shown 1 typical certificate). Page 2 is append page, which including the rest certificate No. (more than 2 certificates in the permission) and CCC mark design pattern.
3. CCC certificate should be corresponding to Permission of Printing/Impressing China Compulsory Certification Mark.
